Request a Certificate of Insurance (COI)
INSTRUCTIONS for youth endurance clubs that have current YES-Athletics-365™ liability insurance coverage:

365 Clubs can request a Certificate of Insurance (COI) when required from the property owner of club practice areas and venues where the club is hosting competition.  There is no additional cost for 365 Clubs to request a COI for their practice locations and venues for competition.

Please allow two (2) weeks to receive a COI from the date it was requested.

To request a COI, please provide the following information in an email to:  [email protected]

Head Coach Information - - - 
(Person requesting COI)
- first and last name
- cell phone number

Venue Information - - - 
- Name of the Venue
- Owner of the Venue
   (This should be the specific name
     that the venue owner requires to
     be listed on the COI.)
- Physical address of the venue owner
     (i.e. - city, town, school district, etc.)
- If the COI is for an organized competition
    provide the date and time of the competition.
